Problem with gears changing up...

I've had more than one instance in the last week where I've changed up and as I've let the clutch out the higher gear has not engaged even though I felt the positive click from the gear shift.

On three occasions yesterday the gear box has remained in the lower gear, on one occasion I recall having to have to select the higher gear again and on the other it jumped into the higher gear suddenly.

Can't recall any issues going down the box

The bike is an 07 model with 4.5k miles

Any thoughts as to what the problem might be?

I'd first make sure that all the shift linkage is tight.
Next, concentrate on whether or not you have a lazy toe. I know at times I get lazy when shifting and find myself between gears especially when going from 1st to second.
 
As mentioned in several past threads, check the gear lever pivot is lubed, free and not binding as this can prevent the shift mechanism from properly engaging the next gear and hence it can jump out of gear back into the one you've just shifted from, or maybe even only shift into a false neutral.
