After 3 years and approx 50k miles finally found what was knocking/rattling in the engine all the time.
Part 5 from microfiche - valve lifter was worn out and had 2mm of play in the "cylinder".
Tested today on motorway at max speed - I could not hear the engine working
Replaced all camshaft assembly with another from RT engine.
