The lever is in two parts, the bit that connects to the cable and the bars and the lever part itself. These are connected by a peg with a circlip underneath. It appears that the circlip has come off (or wasn't there in the first place?), the peg has worked its way up and eventually fallen out. The lever has then gone for a solo tour of Lincolnshire

I have been told that a batch of these levers came without the circlip fitted. You can't tell when they are in the packaging as it is underneath the lever and comes in shrink-wrap. Just worth checking if you have these fitted or are fitting them. I was okay as I was accelerating at the time, but could be a bit alarming if you're trying to slow down rapidly for a corner when it falls off
