Nationwide BS has changed insurers for travel insurance cover with their FlexPlus account.
Aviva has now taken over from their previous insurers.
We recently discussed our cover with them, and found the telephone interview with them as being woeful. The person we spoke to had no clue as to what we were telling him, and as a result, a couple of conditions the previous insurers covered were now subject to a surcharge, and some conditions they would not cover ( one of mild hypertension well controlled with 1 pill), which we were surprised about. The only benefit we could see was that there was no 70 year old age restriction before an automatic surcharge was applied.
If you use Nationwide FlexPlus, just check you are covered.

On a positive unrelated note, the Spanish seem very good at trying to milk private health insurance especially for British tourists at the seaside, but friends of ours who have property in Austria report that the GHIC card has proved invaluable to them. They have had multiple healthcare visits and the Austrian Health system are quite happy to provide treatment on production of a GHIC card.
 
I looked at the Nwide account. I did the medical screening online. Did you try this rather than speaking to a screen reader? I am 62 and they were willing to cover hypertension controlled by more than one pill as well as other stuff. Their top up premium was higher than a standalone annual policy from Voyager's Able2travel broker for the decrepit, so I did not proceed as the overall cost was about double what I could pay for travel and breakdown separately.

The Spanish have been trying to fleece UK insurers for decades. My lad is 34, when he was 9 we had to go to a hospital. Our hotel had arranged a GP to visit him after an accident. He organised an ambulance and we were taken to a hospital. The GP gave us an envelope to hand to the hospital, presumably so he could get a kickback. No interest in the E111 that was current at the time. They insisted I showed them documents and paid the excess rather than accompanying my son who was taken to be x-rayed. Son was not broken, just sprained and had one of those soft casts and a sling, with a follow up X ray back in the UK after the swelling went down.
On returning home I completed forms for the insurer, Fortis (now Ageas). Fortis refunded my excess and paid incidentals for pain meds etc. They followed up seeking further information as the invoice from the hospital was completely out of proportion to the information I provided and they should have recognised the E111 and made a claim to the NHS not Fortis for the emergency treatment.

Interesting regarding other insurers.
My experience of Spain is exactly the same as you have described!
