Use a touque wrench as these have to be done up TIGHT!!. Happened to me a couple of times till I saw the light and was supprised how tight the correct
setting was.
 
IIRC from my 1150 days you part torque the nuts to 70nm and then finish them off at 105nm.

Of course! :blast.

When I reported brake pad failure to VOSA's "Safety Branch" (the friction material came off the backing plate on 2 different pads in 2 days. And I quoted other instances found on the web), VOSA told me that it was not a safety issue but a maintenance issue "because the daily check includes looking at all the pads".

I tried to point out that
A. This is not easy as they are well hidden
B. If car drivers don't have to why should I?
C. Is this a reasonable approach to something so fundamental to NOT DYING?

but they could have cared less. As long as they get their final salary pension.....(not very) civil servants to the last.

Similar experience - driving along motorway, thought I had a puncture however rear wheel collapsed as bolts had worked loose. Fortunately was able to control bike and not hurt. Very expensive repairs - bevel drive, rear caliper, mudguard, brake disc and wheel!
